Your Child’s Orthodontist is Concerned About How Your Son or Daughter is Caring for Their Braces

If you have found yourself on this page it is because your son or daughter has done one of the following:
- He or she is not properly caring for their orthodontic brackets/appliance and has too many broken brackets
- He or she is not wearing their prescribed elastics/rubber bands as directed
Prior to Your Child’s Next Appointment…
It is extremely important that you review all of the information on this page, print and sign the letter contract acknowledging that your child will adhere to his or her orthodontic treatment as prescribed. Have your child bring this form with them to their next appointment.
If you do not have access to a printer, let us know and we will have the form printed and ready for you at their next appointment. Both you and your child will be required to sign the form prior to being seen at their next appointment.
